Our exceptional team of healthcare professionals includes a cardiologist, registered nurses, exercise specialists, counselor and dieticians—all dedicated to making the process of recovery a positive and rewarding experience.

Program Components

Exercise: A personalized exercise program is designed to improve the efficiency and stamina of the heart, as well as overall physical health.

Education: A wide range of information is shared with both patients and families to help develop a more positive lifestyle.

Encouragement: Support of cardiac rehab professionals and fellow participants can help reduce anxiety and renew self-confidence.


Program Features

The Change of Heart Cardiac Rehab Program is composed of a variety of supportive features, including:

  • Closely monitored exercise classes held at convenient times throughout the day.
  • Active family involvement through support groups and educational classes.
  • Individual consultations and group seminars that focus on management of heart disease, diet changes, risk factors (e.g. cholesterol, stress, smoking, high blood pressure) and exercise.

An ongoing program is available to cardiac rehab graduates to support continuing heart health. This includes:

  • A supervised exercise program
  • A risk-reduction exercise program


Program Enrollment

The Change of Heart Cardiac Rehab Program is available to adults diagnosed with coronary heart disease and is designed to assist with individual care and rehabilitation. A physician referral is required and physicians are provided progress reports on a regular basis. Most insurance companies cover all or part of the program costs. Coverage may vary with individual policies and should be checked prior to entering the program. Members of the cardiac rehab staff can provide assistance with confirming insurance coverage if needed. An ongoing rehabilitation program is available to all those who graduate from the cardiac rehab program.
